This letter is in response to a complaint filed by the above named against CopperPoint Mutual Insurance Company (CPMI).Thank you for the opportunity to respondCPMI issued a renewal policy ( [redacted] ) for Massage By *** [redacted] ***., effective September 21,through the cancellation date May 10, 2015.When a policy is cancelled mid-term, due to switching to another carrier, the policy is subject to a shortrate cancellation, per [redacted] on Compensation InsuranceTypically we do not provide this information, as the cancellation date is not always the same date the policyholder called in to inquire.There can be a difference in what we quoted the policyholder and in essence causing more customer confusion.Included with this response, is the short-rate tableThe calculation is the wages/# of days the policy is in effect x x short-rate %.Customer service and satisfaction are extremely important to CPMI and we endeavor to timely resolve all customer complaintsWe apologize for any inconvenience this may have caused Ms [redacted] , by not providing her the short-rate calculation

(Injured worker) Watch your back and immediately get a lawyer, do not trust a word out of the claims adjuster mouth ... get an attorney immediately or better yet reject workmens comp all together and sue your employer in court! Watch out for the lawyer you get as many in workmens compensation litigation are former employees and their loyalty runs deep!!! Also, watch out for the Industrial Commission of Arizona Administrative Law Judges and Claims Department Managers as some are former attorneys of Copper Point and they have a improper influence over most of the ALJs ... watch out!

generated a credit balance on the policy for $411 .28.CPMI did receive a call in June; however, the balance on the policy was not final as the audit hadn't been processed. CPMI would not have known there was a refund due at that time. CPMI did receive a calion July 13th inquiring about the refund and CPMI advised it does take 30 days to process refunds. Another call was received on August 4th and we did advise the policyholder of the 30 day processing time. CPMI does acknowledge that the 30 day window in which refunds are processed, was not conveyed clearly enough. A credit balance report is run for all of our policyholders on the 3rd· of every month and the 30 day window starts from the date the report is run.Due to the miscommunication and the multiple requests, the refund in the amount of $411.28 was processed August 14th.Customer service and satisfaction are extremely important to CPMI and we endeavor to timely resolve all customer complaints. We apologize for any inconvenience this may have caused Ms. [redacted], and hope this matter is resolved to her satisfaction.
