I appreciate the opportunity to respond to the customer in regards to complaint ID#: [redacted] I can confirm that our medical practice was a vendor for the health fair event on 2/9/17. all patients. However, I can discuss the process in general, but again not relating to specific patient(s). Once an individual is interested in allergy testing, they fill out new patient paperwork including but not limited to; personal demographics, medical history, symptoms questionnaire, insurance information, insurance card and picture ID are presented by patient and scanned by Corebella representative. Upon review of completed new patient paperwork by our on-site nurse practitioner determines if allergy testing is appropriate. If appropriate, testing is administered, results are documented and reviewed with the patient by our nurse practitioner and treatment plan is presented. Consent(s) by patient(s) for services are signed by patient. Billing information is also included in the patient paperwork. Follow-up email(s) and/or phone call(s) noted in patient(s) record also. I would like to confirm the following: Corebella did perform services on 2/9/17, Corebella’s patient paperwork states in more than one area our billing procedure for services. At no time, did Corebella misrepresent our services and/or billing procedures. In general, the health fair event itself may have been free for employees to attend. We do not advertise our services as free. Specific to the complaint: We did not make anyone fill out paperwork and receive our services. The complainant also provides inaccurate information in regards to # of patients, billing amounts, billable services, dates, amounts collected, etc. I am available to assist and answer any further questions, if needed.
