I have received six solicitations to complete a survey AND donate towards their cause in the last 30 days. These solicitations come with similar, yet different pre-printed return address envelopes (see below):

In each case they are soliciting to complete a survey AND send them a donation, generally ranging from $14 to $17. I believe this is a scam. Further, there is no obvious way to stop these solicitations. Just think, if each mailing generates only $14 time the six I have received in just one month would equal $84.
