Crooked Oak Family Health

Review: After being a patient at Crooked Oak for over ten years, we moved to Canada and completed a standard form to send records to a new physician. While none of the other physicians we sent the form to charged us, Crooked Oak did. My primary concern, however, is that I was sent a handwritten note indicating that if I did not respond by a given date, then they would send the forms and charge me. I did not receive this paperwork until after they sent my records and charged me. I did not approve the charge, nor did I agree to pay for this service. They sent it to collections. I contacted them, and I was told that it was noted, but the unauthorized charge would not be removed, and they would continue to pursue payment.Desired Settlement: I would like the fee to be waived and for them to have the collection agency cease in their pursuit to collect the unauthorized fee.
