April 24, 2017Dear [redacted] :Please accept this response from Crump Life Insurance Services, Incand its affiliated companies (collectively referred to as Crump), to your inquiry and referenced complaint filed in your office on 4/17/17,Crump has a Service Agreement in place with [redacted] , the company referenced in your inquiry and the complaint letter [redacted] connects individuals seeking life insurance with Crump, an insurance licensed business entityThe remaining purpose of this letter is to respond to your inquiry and the complaint receivedPer your request, I did not include any personally identifiable information in this responseIf additional information is needed, please contact me directly.The complainant indicated concern with Crump's sales associate, Jenny K***, and was also Concerned that her personal information was compromised, Pleased be assured that Jenny K [redacted] is a licensed insurance associate of CrumpAdditionally, as a licensed insurance entity, Crump obtained a signed HIPAA Authorization from the proposed insured in order for us to obtain and disclose medical information pursuant to the client's request to pursue life insurance coverageA copy of the complainant's signed Authorization is maintained in our file, Jenny K [redacted] requested the information as noted in the complaint as permitted under the HIPAA Authorization and this information remains in compliance with our privacy and security standardsCrump has not breached any information received from the complainant.In reviewing our case file, I did receive confirmation that there were calls made to or received from the complainant, These calls included a conversation with Jenny K [redacted] and a separate call from Jenny's Supervisor, both calls relaying to the complainant that she did not meet the criteria for the insurance coverage requestedAn additional call received from the complainant resulted in further guidance from Jenny that the complainant should contact her doctor for additional information.Upon my review of the case file, believe the communication to the complainant was appropriate, including the disposition of the requested insurance coverageI also confirmed that Crump did not breach any personal information related to the request for coverageLastly, I can confirm the legitimacy of the relationship between Crump and [redacted] and confirm the insurance licenses of Crump are in good order,If you need any further information, please feel free to contact me directly at [redacted] @crump.com or via phone at [redacted] ***We appreciate the opportunity to respond to this inquiry.Sincerely,Joy DVP Market Conductcerely,
