To Whom it May Concern,I have attached Ms***'s contract copy, to provide more detail regarding the buyers right to cancel and our general Personal Training cancellation policiesMs [redacted] received a carbon copy of her three month contract at the time of purchaseOn the back of our contracts it clearly states our cancellation procedures: "Member may cancel this agreement at any time after the initial term (for Ms [redacted] months) by sending in a day written notice prior to the monthly dues out limitation, by reason of members failure to utilize the facilities of JF Fitness of Virginia, LLC DBA Crunch Richmond Brook Road." Our memberships and personal training are two separate things, one is a non contract, which allows you to cancel at any time and that is clearly presented to the customer upon initial sign upThe personal training is a contract agreement which is also presented clearly to the customer at the time of sign upAs you can see, Ms [redacted] has initialed and signed her contract (dated 7/04/16) agreeing to a three month contract for personal training Please let me know of any further assistance I can beThank you!

Spoke with Shane H [redacted] he stated that a contact was signed by [redacted] When the issue was brought to his attention he addressed it with her and answered all her questions some were not to her likingHe stated [redacted] told him that she had a medical issue and couldn't continue the personal training and wanted it cancelledMr [redacted] told her in order to do so she would need to provide a doctors note stating she couldn't participate in training for days or moreHe said she didn't bring a doctors note she brought in a note written on a prescription pad from a psychiatrist, he informed her that wasn't the approved document they need it needed to be a note from a physician [redacted] said ok and left the facility and never returned with the appropriate documentThere for leaving her personal training not capable of being cancelled
