On 7/15/this patient agreed to our self-pay agreement since his insurance did not cover our oral applianceOur self-pay agreement ,at the time, is as follows The patient will pay the agreed amount in payments or pay in full It comes with a year warranty where we will repair or
replace the oral appliance at no extra charges to the patient Also comes with a day trial period where if they feel it is not working for them or are dissatisfied with the product they may return it but there is a nonrefundable fee of $because of the custom fit and to help cover the lab costs The day trial period begins when they receive a viable working oral appliance We do not guarantee that this will work for every patient but we do guarantee the workmanship That is why we offer the warranty This patient may return his oral appliance as he contacted us within his trial period He received his oral appliance on 4/2/and he had contacted us on 4/20/ Our self-pay trial period had changed in September of When calling in he spoke with a newer representative that was not aware of our previous policy he had agreed to We will contact the patient to help with his process
On June 16, the customer was contacted and advised of the product being on back order and a refund was requestedA refund was issued for the original amount of $***

It has been six days today and I had still not heard from anyone from the American Sleep Association. I called them and spoke to a representative. She advised me that I would need to return the device and THEN they would refund [redacted] of the [redacted] I have been charged. I will be mailing the device back to them immediately. If they follow through then THEIR RESOLUTION WILL BE SATISFACTORY TO ME. Thank you very much for your help with this company! Had it not been for your professional assistance, I don't think I would be seeing anything back from them!Very Best Regards, [redacted]
