The dental office is under investigation and a grievance appeal is in process because the dental office refuses to comply with the insurance guide lines. The information stated in the letter is false. If further information is needed I have proof to provide re: the guide lines of the dental insurance along with the grievance complaint.

 That PDF of what the office includes is according to there office policy, not according to the Ada policy. As per the second opinion I received by a lab technician; the partial was made poorly. Impressions were retaken to see the mistake. Where the impression was poured and fabricated the placement of the partial did not seat properly. So it's on the behalf of their lab technician and the dental it was there poor decision of approval on a partial they delivered and knew it was made improperly. As I went numerous of times for adjustments for where my second opinion explained why I needed, because it wasn't seating.

Review: I have a set of partial that are less than a year old. Recently my partial broke in half while cleaning them. I went to the dental office the next day and said I would have to come out of pocket for a repair. I consulted with my daughter who is in the dental field and mentioned to contact the insurance for the coverage warranty. I consult and was told there is a repair coverage every 12 months. I returned to the office and the refuse me service and repair. I filed a grievance complaint, as I was told to by the insurance incase I was denied service; but until then I am left with no teeth to eat properly with. I want my insurance reimbursed so I can repair or replace what the dental office is responsible for doing.Please can someone contact me Thank you[redacted]Desired Settlement: I will like a refund back to my insurance to repair or replace my partial dentures.
