Thank you for your letter alerting us to the problem you had with our facility at DJackson. In response to *** ***'s complaint, we understand the following*** ***, her daughter, signed up for a year membership on 1/25/She was then charged on the same date for the
subsequent months, February, March, April, May, June, July, Aug, and Sept on the 25thWe received an email on 10/12/which I have attached from *** ***This was the first time DJackson was notified by email or phone that *** was no longer hereOnce we were made aware of *** being gone we immediately cancelled her membership with no cancellation feeNo further charges have been made since the phone call and email to DJackson from *** ***We have attached a copy of our standard membership form with the proper cancellation policyWe need an email sent to *** with your notice to cancel and whyHence why once we received an email from *** ***, we cancelled the membership immediately

I was referred to D1 sports training by a family member. She stated they were hosting a 2-month weight loss contest. She provided me with the phone number for [redacted] with D1 Sports whom I called and asked several questions regarding the contest, including the cost involved. [redacted] stated the contest was 2 months in length and the cost was $150 per month. The first day of the contest was Sept 16, 2013. At the onset of the contest, there were no membership papers signed stating any specific payment policy. After the first month of the contest, I decided I could no longer continue due to physical pain and personal family matters. I stopped in on Monday, October 14, 2013 to pay [redacted] for the month I had participated. I explained why I could no longer continue and expressed my thanks for my time spent in the contest. At this time [redacted] stated he was sympathetic to my situation but that I was obligated to pay for both months, whether I participated or not. I did not reply as I was very upset. I handed him $150 in cash for the month I participated and left telling him I would be in contact about the so-called remaining balance. I am disputing the remaining balance of $150 because I was never told I was required to pay for the second month, and also because I never signed a single piece of paperwork. I believe [redacted] is unjustly trying to get me to pay for services that I will not receive.
