Review: I received services on 4/1/15. At the end of my appointment I was advised that the Doctor's office does not have a billing department. I paid for my appointment in full ($120) and was given some paperwork to mail into my insurance company for reimbursement. I was assured that reimbursement would be "no problem". I mailed the paperwork on 4/2. On 4/11 I received a letter from my health insurance, [redacted], stating that the paperwork I mailed in was insufficient and requesting a copy of form [redacted] or [redacted] from my physician. I called the office several times, but the office manager never called back. On 4/18 I mailed a letter in the US Mail requesting documentation. On 4/21 I called the doctor's office and got their email address: [redacted] and emailed a copy of the letter from [redacted] and requested the forms.On 5/8 I followed up with another phone call and email.On 5/27 I went into the doctor's office and left another copy of the letter.On 6/2 I went into the doctor's office and asked the receptionist to have someone contact me regarding my request.I have received NO RESPONSE to any of my requests.Desired Settlement: I want my Doctor and his Staff to be responsive to his patient's requests. I want to be reimbursed $120 from my healthcare insurance provider, but in order to do so, I must receive a response which includes all of the forms and documents requested by [redacted] from my doctor.I want an explanation and apology for the terrible service I have received.
