Review: I am applying for a employment which requires a [redacted]. This application process requires a documented DOT drug test with the required signatures and copy 2 of the DOT drug testing custody form. I was told it would take one week from this company and it has been 4 weeks, and they have not produced any form yet. I have been in contact with their office almost daily and I keep getting different stories on why they have not received the paperwork. Three weeks ago I was told it would be there by the end of the week. Then I was told that they were faxed a copy which to this date has not been produced. They then told me that the fax was not clear or it was blurred and cut off. To date they have to produced the product which was guaranteed me. I paid $65 in cash with no results. I have decided to use another drug testing facility and they had all the paperwork and required information back to me in less than 24 hours.Desired Settlement: To refund me in full, the amount I paid them, since they did not produce any documentation.
