After speaking to salon management, we have no record of a cancellation from Mr [redacted] We have no cancellation form on file from when he stated he cancelled his membership.As per the cancellation section of the membership agreement it states the following:1) Only L.ATan VIP cancellation forms will be acceptedNo Exceptions.2) Memberships must be cancelled in person at the store that bills the monthly payment.It also states the following:L.ATan to automatically charge $to my credit/debit account, checking account, or savings account for payment of my monthly VIP membership feeThis authority is to remain in effect until L.ATan receives a VIP Cancellation form filled out correctly, signed on premise and countersigned by managementManagement will not countersign your cancellation form if it is incomplete or not filled out correctlyI have attached his signed membership agreement for the recordThe most we can offer Mr [redacted] is a one month $refund or send him a $gift card to be at his convenience
