Ms. [redacted] (complaint ID # [redacted]) Dear Ms. [redacted]: Human Arc very much regrets the concerns Ms. [redacted] raised to the Revdex.com relative to her experience with Human Arc. We sincerely apologize to her for any inconvenience that she has experienced. We cannot stress enough that it was never our intention to create difficulties of any kind. Based on the complaint submitted by [redacted], we have looked into her file to determine the process that was undertaken to attempt to provide her assistance with obtaining Social Security Disability Benefits. We work in collaboration with health plans to provide eligibility enrollment assistance to members who we believe might benefit from our services. Upon reviewing her file, we can unfortunately corroborate [redacted]'s statement that she did not speak with a representative from our company. Our associate spoke with another member of [redacted]'s household (an unidentified male), who indicated in the conversation that while he did not believe [redacted] would benefit from our services, it was acceptable for us to send further correspondence to [redacted] in the future. This was not appropriate action on the part of our associate and we deeply apologize to [redacted] for the confusion and misunderstanding. While we did receive [redacted]'s name and information from her health plan, we can advise her that per her request to the Revdex.com, we have removed her information from our system and will not contact her in the future. Human Arc is a firm that is hired and authorized by healthcare providers to offer cost-free screening for individuals for their potential eligibility for government-funded healthcare coverage programs and then, if potential eligibility seems likely, we help healthcare consumers, should they choose, throughout the application process to secure that assistance to receive benefits. We have won numerous awards for our civic involvement and community service, and we are renowned in our industry for our work in making America healthier through our commitment to, and compassion and respect for, the patients of our client hospitals and members of our client health plans. We are very proud of the work we have been doing for over 30 years for hundreds of thousands of individuals, and we welcome any feedback, positive or negative, that may help us improve our service. We take complaints from the people we serve very seriously. Again, we apologize for her concerns and can again assure her we will not contact her in the future. Very sincerely, Jenny R[redacted] Compliance & Legal Services Manager 1457 East 40th Street, Cleveland, OH 44103 [redacted], ext [redacted] fax 1457 EAST 40th STREET, CLEVELAND, OHIO 44103 • [redacted] • [redacted] • FAX [redacted] www.humanarc.com

Initial Business Response /* (1000, 5, 2017/03/15) */
We received an email from our dealer Doors Galore on a warranty claim from [redacted] in [redacted] The email included the attached two pictures stating there was rusting on the bottom section. We reviewed the attached photos and spoke with the...

homeowner and dealer and found that the rusting was localized to the bottom section, on the bottom on the track and to the bottom section's hardware.
The consensus and reasoning was that road solvents used for snow and ice removal from the vehicle coming in and out of the garage and the vehicles being started blowing the solvents onto the bottom section of the door from underneath those vehicles, pooled and had built up on those areas over time caused the rusting. The exhaust may have also contributed to the issue over time.
The reasoning considered that being that all of the other sections and hardware shows no sign of the surface rusting. All of the sections were made at the same time from the same roll of steel and they show no signs of rusting like those areas exposed to the vehicle(s) exhaust blowing the road melt onto the door, hardware and track.
Even after the review was performed with the information supplied, Safe-Way Door took great expense and time to go to the home and review the door and conditions in person to make sure that all factors were considered. Upon personal inspection the findings were confirmed. Mainly from the localized area of rusting and to the other components on the door that were manufactured by other manufactures (not Safe-Way) showed significant corrosion as well.
Our warranty, see attached, only covers rust through due to the paint system cracking, checking or peeling thus exposing the steel underneath to open air. It doesn't allow for substances getting on the door causing rusting from the outside down into the paint and steel. Our warranty is included in this document and is an industry standard warranty that all garage door manufactures follow.
If the homeowner would have called us of the dealer, when they first noticed the spotting caused by the substance, we could have instructed them on how to properly maintain the door and it would have been resolved and the rusting lessened. The reasoning for denying the warranty claim is supported by the Warranty exclusions of foreign substances and improper maintenance. Thank you.
Initial Consumer Rebuttal /* (3000, 7, 2017/03/21) */
(The consumer indicated he/she DID NOT accept the response from the business.)
It doesn't surprise us that Safe-Way is not honoring their warranty agreement after meeting up with their representative. We disagree with their response and do not accept it. The biggest portion of the rust is only on one side so how do they explain this? We have two vehicles that have always been in the garage. We don't start our cars with the garage door closed. We take pride in our home and to state there was improper maintenance is not true. We also contacted Safe-Way 2-1/2, 3 years ago about the rust and their response was that it had to rust through before anything could be done. It's unfortunate that we did not document the date and who we spoke with concerning this. If this isn't resolved to our satisfaction, we will not be purchasing anything that is manufactured by Safe-Way and we'll be sure to let our friends, family, etc. know of this. Please do not show this as being resolved through the Revdex.com.

October 17, 2016 Ms. [redacted] Dispute Resolution Specialist Revdex.com, Inc. 2800 Euclid Avenue, 4th Floor Cleveland, OH 44115 Re: Complaint submitted to Revdex.com about Human Arc on 10.11.16 by Ms. [redacted], (complaint ID # [redacted]) Dear Ms. [redacted]: Human Arc very much regrets the...

concerns Ms. [redacted] raised to the Revdex.com relative to her experience with Human Arc. We sincerely apologize to her for any inconvenience that she has experienced. We cannot stress enough that it was never our intention to create difficulties of any kind. Based on the complaint submitted by Ms. [redacted], we have looked into her file to determine the process that was undertaken to provide her assistance with obtaining Social Security Disability Benefits. As she stated in her letter to you, we are a vendor of her health plan and with their collaboration, we provide eligibility enrollment assistance to members who we believe might benefit from our services. While she accurately states that communications she's received from the Social Security Administration (SSA) advise that as an Authorized Representative, Human Arc is eligible to receive a fee for our services, we have not requested a fee in Ms. [redacted]'s case, nor did we ever have an intention to based on our contract with her health plan. Correspondence created by the Social Security Administration is solely their works and Human Arc has no influence on the language that appears in them. We are disappointed to hear that Ms. [redacted] was unhappy with her experience with our organization and feels we did not provide the best customer service. Again, we apologize for her concerns that she was not treated well by the associates in our organization and can again assure her that we have in no way requested compensation for her case.
