Review: I was diagnosed with rheumatoid arthritis and after learning that my doctor would be moving out of state decided to look for a new doctor I found DR. David Neustadt at which time I made an appointment the lady who answers the phones up front and scheduler [redacted] rescheduled my appointment 4 times the last time she called she told me the wrong appointment date and time and now they are trying to bill me for services that were never performed and I am not a patient there nor have I ever been to their practiceDesired Settlement: bill taken out of my name

To confirm our telephone conversation with RE: the complaint information form on “[redacted]", I am sending you this note and information. He first called our office on July 20, 2011 for an appointment for a “second opinion". He was not directly referred by any physician. Apparently, someone recommended that he see me for a rheumatologic evaluation for his 2nd opinion as to diagnosis and treatment. My receptionist, who has worked for me for 22 years talked with him and gave him an appointment. Subsequently he called to reschedule the date first given. Similarly, after this, he called and asked to change his appointment a 3rd time. Finally, after a 4th appointment date, he was reminded of his appointment the day before, but did not keep this appointment and did not call to cancel. He was informed (in our mailing packet for new patients) of our office policy to cancel in advance with 24 hours notice or that he would be billed for a no-show fee of $25.00. After this event, he was billed the $25.00.

I am enclosing a note written by Ms. [redacted], my office manager which I think summarizes the final details of this complaint by Mrs. [redacted].
