Once again, we contacted the patient and let her know we would refund her payment if she came into the office and signed a release form. She once again e-mailed on 09/16/2015 and asked that the form be sent to her. We replied and let her know the form needed to be signed in person, in our office. We do NOT mail the form out for signature or review. She is more than welcome to come review it in our office before signing.
