Davis Vision (Guardian)

Good afternoon, I went to [redacted] in Pikesville, MD on January 27, to discuss having some sun glasses that I purchased the week before converted to eyeglasses I was told that my glasses could be changed into eye glasses and I was told that my sun shade lenses would be kept and sent back to me/ [redacted] with my glasses My glasses were sent to them, Davis Vision, and received on February 8th When my glasses were sent back to [redacted] for me to pick up, they arrived without my shade lens I was later told by the staff member at [redacted] that Davis Vision threw them away Since then there was a sorting out between Davis Vision and [redacted] as to the replacement of my lenses They were [redacted] lens, I expected the exact lens to be replaced I was told by [redacted] on Tuesday February 20, that Davis Vision was going to replace them I waited two more weeks and heard nothing I contacted Davis Vision directly and the customer service repinformed me

claim for reimbursement of contact lenses, per policy benefit of $per year for contact lenses
Davis Vision May 1, claim denial letter stated that I am not listed in their files as a patient
One month prior to this letter I spoke to Davis Vision by phone, a Davis Vision employee provided my Davis Vision policy ID number to me
My Davis Vision ID # and my social security number, along with full name and address were provided on the Davis Vision claim form
In and Davis Vision only paid half my claim until I contacted Davis Vision to request the full benefit reimbursement
Apparently Davis Vision is engaging in a strategy of denying claims in order to maximize profits
Potential Davis Vision customers should be aware of this practice

On Nov,22,I went to have my eyes examined at VISION WORKS in Bartonville ,P.Aafter sending Davis Vision a co-pay of $to cover the exam and biofcal lenes if required.My exam results was NO CHANGE TO MY EYEGLASSES PRESCRIPTION for this yearI called Davis Vision to inform them that I only received an eye exam and had also pre-paid for lenesSo I was requesting a refund
The rep put me on hold and called VISION WORKS to verify my statementShe returned to the phone and told me I was to receive a $70,refundThat was on the same day of my exam at 4:pmI have not received a refund yet even though each rep I have spoken to verifys iam entitied to one
The date of the calles concering this matter are as followes, 12/29/17,1/25/18,1/30/18,02/13/(,this call was I even given a check number of #*** sent out on the 1st of FEB and I should receive it by the 22nd of feb.2018)NO CHECK.so I called back on 2/23/only to find out that there my have been an add

My wife (***) ordered new glasses on Nov3rd I ordered new glasses on Nov6th The glasses were ordered through our eye doctor (DrMitchell F* of Willingboro, NJ and/or *** *** *** of Willingboro, NJ) We were told by Amanda (from the eye doctor's office) we would have our glasses in 7-days On Nov21st, we have not heard or received or glasses I called Amanda, who contacted Davis Vision Davis Vision (Julie) told Amanda my wife's glasses were made incorrectly The lenses did not fit in the frame because the lenses were cut for a different pair of frames Julie told Amanda that the lenses were in the process of being polished Amanda spoke to Brenda (Davis Vision) about my glasses Amanda was informed my frames were lost and no further information I called Davis Vision and spoke to Donna Donna indicated my wife's glasses were shipped on Nov20th She was not able to provide any further details on my glasses I then spoke to Supervisor Bella Bell

Davis Vision is my vision insurancewith that being said, I ordered glasses in the middle of June It is now The beginning of August and I still do not have my glasses!!!! When my glasses were ready for pick up, I noticed that there was a scratch on the left lens My glasses were returned the same day I picked them up After two weeks of waiting, I was told that my glasses were to be shipped on 7/27/2018; however that was not the case After calling back to Davis Vision to check the order of my status I was told that my glasses were LOST and prior to them being lost, they failed inspection times After talking to customer service rep Chris, I was told I needed to submit my receipt and then contact the place where I ordered my glasses, so that another frame could be sent out After speaking with Chris, I asked to speak with a supervisor who's name was Lauren I began to tell Lauren how upset I was at the fact that its been going on two months and I still do not have my
