This is in response to a
complaint that was filed against us by [redacted], case #[redacted].  He
came into our office on 11/**/2013 for a root canal on tooth #29 with an
evaluation, x-rays and a post and core.  He was quoted $1,673.00 for
these services.  His out of pocket according to the insurance would
be $927.00 and the insurance estimated that they should cover
$746.00. 
[redacted] decided to
only have the exam, x-rays and pulpotomy done that day.  Those services
came to $193.60.  We collected $18.40 from him this day and submitted the
claim to the insurance.  The insurance only paid $73.60 which left a
balance of $120.00. 
[redacted] signed
documents explaining to him that the fees quoted from us and the insurance
are only estimates, that he was responsible for the balance that the insurance
does not pay. 
[redacted] came back
into our office on 12/**/2013 to do the root canal on tooth #29.  He did
not want to do the post and core this day so it was estimated to him that for
the root canal only, the fees would be $1095.00 and his out of pocket was
estimated at $579.00 and we would expect $516.00 from the insurance.  The
insurance paid $516.40 and the patient paid us $500.00.  That left a
balance of $78.60. 
Now with the $120.00 he
owed for the exam and the $78.50 for the root canal, his balance is now $198.40
that he owes us.
On 2/**/2014, [redacted] came back into the office to do the post and core on tooth
#29 which was quoted to him on 11/**/2013 to be $390.00.  He
also wanted to get another root canal on tooth #28 with a post and core on
tooth #28 as well.  We did not collect any money this day and submitted to
his insurance for a root canal 28 for $616.00 (Gave him a $479.00
discount because our fee is $1095.00) And we submitted a post and core on tooth
#28 for $390.00.  We did not even charge him for the post and core on
tooth# 29 which is another $390.00 discount.  His insurance paid $516.00
for tooth #28 which left a $531.20 balance.
These fees were quoted
to the patient and he signed them to acknowledge.  We have copies of these
as well.
