completely• Concern Member *** ***'s son did not show up as having medical coverage in Independent Health's system and member had to pay out of pocket for his appointmentMember has called and emailed several times for reimbursement with no resolution• Resolution- Upon investigation, Independent Health learned that there was a problem with the file of this member's son during our conversion to a new claims system which caused an eligibility discrepancyOnce corrected, the claim for the member's son was adjusted so that it would pay the physician correctlyHowever since the son's primary care physician was not listed in the system, the claim was billed incorrectly againOnce this problem was identified and the son's physician was listed in our system, the claim billed with the correct $co-payThe reimbursement check to the physician was sent out on June 10, Independent Health reached out to the physician's office to ensure that once they receive the reimbursement check, they need to forward the reimbursement amount (minus the$co-pay) to the memberIndependent Health will follow up with the office to ensure the check is mailed to Ms***• Concern- Member was disputing her $co-pay for her OB GYN appointment, as she felt it should have been considered a women's well visit with a $co-pay• Resolution -Independent Health confirmed that the member's medical plan has a $routine physical co-payWe have also verified that while certain services such as pap smears are covered in full at $co-pay, if a routine physical is included, the visit is billed with a $co-payWe also confirmed that well child appointments for children under age are covered with $co-paysWe encouraged the member to obtain a summary of plan benefits from her employerLastly, Independent Health researched the member's concern of a lack of an email response and discovered a system issue which prevented the email response from reaching Ms***. This system issue has since been corrected. Independent Health has reached out to Ms*** and she has been provided with this information along with an apology for the inconvenience and a promise for a follow up call to the physician's office to ensure that her reimbursement check is sent to herThank you for bringing this situation to our attentionThis member's experience is not the norm at Independent Health, where our commitment to service has earned us national recognition of our customer serviceAll feedback that we receive is used to help us continually improveSincerely, *** *** VP, Health Promotion & Wellness Independent Health

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below. I have spoken with a representative from Independent Health on this issue and I am still not satisfied with the outcome. I understand it was all billed correctly, and I was told they were not going to change their decision and that I should pay the amount so as not to have any credit issues down the road. I have no choice but to pay to keep my excellent rating with the credit bureaus. I do believe that they should have explained in better detail and with examples, on the billing of certain testing and not hind behind badly worded summary sheets. I just am not happy, and if I could I would cancel my plan with them, unfortunately I am not allowed through my employer to do so. Thank you for all your help in trying to resolve this matter, unfortunately, big business wins again.  Regards, [redacted]

This letter is the response from Independent Health to Revdex.com complaint ID #[redacted] from [redacted] A. [redacted] dated October 23, 2015 which was sent to [redacted].  The concern of the complaint from Mr. [redacted] was reviewed and the following steps were taken to ensure Independent...

Health addressed it completely. • Concern- Member [redacted] was billed for bloodwork/laboratory testing that he had done at the direction of his doctor that he understood to be covered at no cost to him based on plan and benefit information that he received from Independent Health.• Resolution -Independent Health reviewed the summary of benefits chart that Mr. [redacted] would have received during open enrollment to determine if the language regarding out of pocket costs for laboratory testing was accurate. It was confirmed that the chart accurately reflected that Mr. [redacted], being in a high deductible plan, must meet his deductible for laboratory tests under diagnostic testing services, after which point the services have a $0 copay/visit. It was also confirmed that certain preventive testing is covered in full with no out-of-pocket costs to the member if performed by an in-network provider. A review of the claim information for Mr. [redacted]'s laboratory tests showed that the 3 preventive tests he received were correctly billed with no responsibility to the member. The other 8 prescribed tests, which were not preventive, were also billed correctly against Mr. [redacted]'s deductible. An outreach call was made to Mr. [redacted] to explain how his benefit works with respect to the lab tests and his deductible. In addition, information on which lab tests qualified as preventive and which ones were billed against his deductible wasalso provided. It was recommended that Mr. [redacted] consult with his doctor prior to future lab testing to determine the tests that would qualify as preventive and the ones that would not qualify so he has an understanding of his out of pocket costs. Thank you for bringing this situation to our attention. All feedback that we receive is used to help us continually improve. At Independent Health our commitment to service has earned us national recognition of our customer service and our goal is to continue to be a leader in this area. Sincerely, David C[redacted] VP, Health Promotion & Wellness Independent Health

I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this does not resolve my complaint.  For your reference, details of the offer I reviewed appear below. I will not consider this resolved until I receive my refund. Additionally, Independent Health should change their advertising materials and REMOVE "$0 co-pay well woman visits" if they themselves do not offer that.  I also do not feel I have been compensated for all of the time and effort I have had to invest in TRYING to correct THEIR serious of errors, as outlined by the company representative below. Regards,  [redacted]
