This is a response to Ms. [redacted]s letter. On Dec 4,2015 Ms. [redacted] came into our office with her mother. She had a letter appointment then her mom. So we tried to see both of them at the same because they were sharing a ride to our office. She had lost her partial lower denture. She was told...

about the snap on smile. She agreed to buy the snap on smile. Both Dr [redacted] and the assistant had explained to Ms. [redacted] about this dental product. It take 15-20 mins alone just to do an impression on any patient's mouth. Accusing Dr. [redacted] of spending only 5 mins with her is a false statement. She was told that we would be sending the impression out to our lab in Southern CA and it would take 2 weeks to get it back.During the time of billing Ms. [redacted] gave us her Care Credit Card to buy the snap on smile. We ran the card online with care credit. It was approved. When we tried to print the statement our printer kept on jamming up. I told her I had to have her sign the approved paperwork it was part of my responsibility. She has to be somewhere else and need to get going. Ms. [redacted] said she would sign it when she came back to pick up the snap on smile. So I let her leave our office she wouldn't be late to where she was going.Dec 17, 2015 Ms. [redacted] came in to pick up her snap on smile. It fit perfectly. She was not happy with what she received and wanted a refund. Dr. [redacted] told her we would be charging her a 25% for a return/ lab fee for materials used to maker her snap on smile. She agreed to pay the 25% fee. We had her sign an Agreement and Release form before we would issue her a refund. Ms. [redacted] read the agreement, made a note on the agreement letter and then signed it. At no time was any fraud committed. There were times when Ms. [redacted] requested money to be refunded or treatment be changed and altered for submitting to the The CA Rehab. We had informed her that this would be fraud and we could not proceed. Our office went above and beyond  to see Ms. [redacted] and worked with her financial situation at the time. We have refunded Ms. [redacted] many times since she started reciving services from our office. Inclosed is a copy on Ms [redacted]s patient ledger and copy of The Agreement and Release form that she signed. Thank you, Dr. [redacted]

Due to federal privacy laws, we cannot provide a response to the complaint. Should Ms. [redacted] provide a signed HIPAA release, we will be happy to respond.
