Moweaqua, IL 62550 I am writing in regard to the above referenced complainant regarding billing for ambulance service provided [redacted] The complainant is the mother of the patient (who is a minor). The complainant has signed the necessary document so I am able to discuss protected health information with you, a third party. Services were provided and information received to bill [redacted] primary and Professional Benefit Administrators (PBA) secondary. [redacted] applied the billed amount of $892.60 to the deductible and did not reimburse Decatur Ambulance Service. PBA did not reimburse Decatur Ambulance Service, as secondary insurers commonly do not pay if the primary insurer does not pay. I have attached copies of both the [redacted] and PBA Explanation of Benefits. Please note that the redacted portion of the [redacted] EOB pertains to an unrelated account. The complainant was mailed account statements on 28JAN16; 03MAR16; 07APR16; and 12MAY16. Further, the complainant was mailed statements with collections warnings on 16JUN16; 20JUL16; 24AUG16; and 12JAN17. The account was turned over to [redacted] Collections on 02MAR17. Complainant contacted our office on 15MAY17 at 1142 hours. Call taker notes complainant acknowledged receiving said statements. Further, complainant was advised that insurance had been billed and she would need to contact her insurance company to see why they had not paid.
