Who's provider was charged for Fluoride treatment?
Sorry for the missed confusion, I talked to my attorney
about the situationWhat he said is the company seen that it was going to my insurance
company, so they in turn raised the price of the procedures so that
they’ll get
paid more that what typically costIn doing this the company tried to bill my
insurance company but used my social instead of my DOD benefits I.Dnumber
which the insurance company didn’t recognized so the bill came to me insteadI
tried and call to get this issue sorted but attempts had failed either they
where closed except for emergency dental only or at some convention
Also was that fluoride treatment not given to your child?
The fluoride treatment was not given to my child LunaAs the
dental office said was that she didn’t need the treatment

Our prices are set according to the average in the area and are registered UCR fees (Usual, Customary, and Reasonable). The prices were not raised more than what they typically cost to, get paid more, it is what the office charges to everyone. Members have the duty to choose from in network providers and out of network providers. We are out of network providers with his insurance provider, so we are entitled to our own UCR fees but even his insurance benefits are higher than what he is stating. Being out of network with his insurance we have the right to charge the patient the full amount up front. As of the writing of this response, Neither the insurance company, nor the patient have paid anything to this office. We have not sent a bill to the patient. We billed the insurance and what we believe he received was actually an EOB (explanation of benefits), The patient has not been sent a bill from the office, and one was not going to be sent out until the issue with the insurance provider was resolved. All procedures were done including fluoride treatment for the patient, On January of 2018, he called the office aggressively and berated the staff. Attached is a signed form where he accepts the responsibility to the account.
