I have reviewed the response submitted by the business and have determined that the response does not satisfy or resolve my issues and/or concerns in reference to complaint # [redacted]. Please add your rejection comments below. I did receive payment from Delaware Life on March 17. The amount was automatically deposited into my checking account. Then on March 17 (the same day)  the amount was  withdrawn by Delaware Life. Then on March 21, 2016, Delaware once again deposited a payment into my checking account. Then on March 22, 2016, they withdraw the same amount from  my checking account.  The issue has not been resolved because Delaware has withdrawn both payments from my checking account. I did receive a letter from Delaware dated March 18, 2016, where they indicated that they had successfully processed my March payment and that my next payment is scheduled for April 1. Unfortunately the money I received from Delaware was withdrawn without my knowledge.. This has caused emotional stress and a financial hardship on my family. I believe Delaware is acting in bad faith by withholding my annuity benefits.

not listed as beneficiaries. The designated beneficiary for the contract is a trust. To date, we have received a Trust Verification Form signed jointly by the appointed Trustees. However, the following paperwork remains outstanding. Form W-9 for the Trust signed jointly by the appointed Trustees.Distribution at Death Option Form completed on behalf of the Trust and signed jointly by the appointed Trustees. Upon receipt of these documents in good order, we will be happy to remit payment to the Trust for the claim proceeds. if you have any questions, please contact me directly by phone at [redacted]

to June 1 of 2015. Ms [redacted]’s monthly payments of $99.64 began on July 1, 2015. We regret the delays that Ms [redacted] encountered, but we did not have an up todate address for Ms [redacted], nor had we been informed of her name change. Once contract was established, it required several written and verbal communications in order to secure accurate information and proof of identity. Copies of our correspondence are enclosed.
