In response to the complaint made by Mr [redacted] on May Mr [redacted] did come into our clinic at [redacted] on Wednesday April 9th, 2015.Mr [redacted] scheduled an appointment as Limited visit for a Partial Denture exam Access dental tried to performed an extensive oral exam during his visit Due to severely compromised upper and lower teeth we were only able to perform the extensive exam and substituted with a limited exam and X-rays Dr [redacted] reviewed the finding with the patient Due to severe calculus, plaque and loose remaing teeth the progonis to fabricate and stabilized the arch for retention of the denture would be complicated Dr [redacted] advised patient that it would be beneficial to get a second opinion by a Prosthodontist who is a specialist in these kind of cases Patient was having a difficult time understanding the medical/dental evaluation Mr [redacted] just wanted the partial to be done regardless After servral attempts to explain to Mr [redacted] the pros & cons, Dr [redacted] suggested a Full Mouth Debriment (Deep Cleaning) to try and remove the calculous and plaque for a better attemptMr [redacted] refused and only want the Partial Denture to be doneHe also agreed to take the referral to the Prosthodontist On April 13th, Mr [redacted] returned to our SW White office demanding he wanted to get his partial doneHe stated he tried to call UTSA Prosthodontist and no one answered Dr [redacted] explained to the patient again that he needs to go to a Prosthodontist or he can call his insurance for another site if UTSA could not assist himDr, [redacted] did take a couple of pictures to visually show the patient of the plaque and calculus that does not come out in color on xrays He did not want a deep cleaning performed and he refused to seek alternative options On April 16th, Access dental called and informed patient clinic will be closed for remoldingLeft message on vm on his phone If you have any questions or I can be any further assistance, please call me at the below number [redacted] Regional Director SA & Corpus Christi
