This letter is in response to a complaint ID *** submitted on 12/17/12:00:00AM by our patient *** *** ***.*** *** *** came as a new patient to our office on April 24, for exam, cleaning and x.raysOn that visit we checked her insurance benefits and she
had an individual active plan that was self-fundedShe was diagnosed with some caries lesionsBased on her insurance benefits we created a treatment plan for her with the estimated amount she would have to pay for her restorative work, but she decided to defer the treatment at that point.As a courtesy, all dental and medical offices routinely asked returning patients if there are any changes in their insurance coverage*** *** returned for her routine cleaning and exam on Nov 18, At that visit, we asked her if there were any changes in her insurance policy and she claimed there were no changesAfter the cleaning and exam, she scheduled an appointment to have her restorative work done on Nov We based the amount charge on her insurance benefits from her previous visitWe took *** ***'s words on good faith and performed the exam, cleaning, x.rays, and fillings for $totalWe gave her an estimate of $as her portion and an estimated insurance portion of $427.We sent the claim to her insurance and later we received an explanation of benefits denying services because the insurance had been terminated on July 31st and patient never informed us of the changePatient had an appointment for adjustment on Dec and she was informed of her balanceSince it was an individual plan, we suggested her to contact her insuranceWe also offered payment plans to help her with the paymentPatient refused to make any payment arrangement and left the office without getting the adjustments done even though we reserved a specific time spot to see her.We followed up with *** *** later on that same day because we were concerned about her discomfortShe requested to speak to the office managerWe informed her that this is a small business with only employees and we don't have an office manager but we have two designated employees that deal with insurance claimsWe assigned our insurance coordinator that speaks fluent English to take care of *** *** concerns on Dec 15th at 12pmAfter speaking to the coordinator *** *** claimed she understood she was responsible for herbalamce and paid the balance in fullThe coordinator spoke and answered all her questions and the dentist did her adjustment at no charge the same day and left in a good term.I hope this letter clear out any misunderstanding please don't hesitate to Call us with any questions
