2016, a renewal letter was mailed to Ms*** identifying the premiums for
the upcoming plan year
(enclosure #1)This renewalletter was sentto the address on file for Ms***, which matches the address providedunder 'Customer Information' on the complaint.Per the terms of the Individual Dental
Policy (enclosure #2), in the event DDOK determines a change in the rates is
necessary, advice of such change must be given to the Policyholder, in writing,
no less than thirty (30) days prior to the effective date of such changeDDOK
upheld this requirement by delivering the renewal letter sixty (60) days in
advance of the effective date.In addition to the
renewal letter, Ms*** was sent an auto-draft email reminder with the new
premium amount on December 25, 2016, two (2) days prior to the December 27,
draft of her January premiumThiscorrespondence was delivered to the emailaddress on file,which matches the email address provided under 'Customer Information' on the complaint.In an effort to resolve this matter, DDOK is willingto make a one-time exception to move Ms*** into another plan of her choice effective January 1, 2017.DDOK will alsoagree to applythe difference of the
January- May premium paymentstowards future premium paymentsunder her new plan.DDOK requests that Ms*** contact
*** ***, Supervisor of Eligibility and Electronic Services, at
405-241-on or before Friday, May 19, in order to

plan option was being discontinued effective December 31, 2016 (enclosure #1).
This renewal letter was sent to the address on file for Mr. [redacted], which matches the address providedunder 'Customer Information' on the  complaint.Per the terms of the Individual Dental
Policy (enclosure #2), in the event DDOK determines a change in the rates or other termsand conditions of the Policyis necessary, adviceof such changemust be givento the Policyholder, in writing, no less than
thirty (30) days prior to the
effective date of such change. DDOK upheld this requirement by delivering
the  renewal letter sixty (60)  days
in  advance  of 
the  discontinuation date.Mr. [redacted] electeda new plan effective January1, 2017. Upon review of call records,Mr. [redacted] did not consultDelta Dental regarding orthodontic coverage underhis new planselection prior to enrollment. A Declaration Letter and new
Individual Dental Policy were issued on December 28, 2016 upon receipt of his application. (enclosures #3 and#4)Per the terms of the new Individual Dental Policy
(enclosure #4), orthodontic
benefits are only allowed for medically
necessary orthodontic services to help correct severe handicapping
malocclusions caused by cranio­ facial orthopedic
deformities involving the teeth, such as cleft palate and other genetic
disorders.In an effortto resolve thismatter, DDOK is willing to make a one-time exception to move Mr. [redacted] into another plan with traditional orthodontic coverage
effective December 1, 2015. DDOK will also agree to apply the difference of the December 2015-June 2017 premiumpayments towards futurepremium payments underhis new plan. DDOK requests that Mr. [redacted] contact [redacted]
[redacted], Supervisor of Eligibility and Electronic Services, at ###-###-#### on or before
Monday, June 19, 2017 in order to execute this one-time plan change outside of the
standard open enrollment period.Please
