April 18, 2013 I went to have Botox, and this is my second visit to **. [redacted] for Botox.. the first Visit and my first time with him he gave me 40 units of Botox and it' Ok.. Wondering why he did not take a picture first and access me.. Usually since 2008 I going for botox they will be consultation and doctor will assess how much units he has to give ( so I know the price and agreed to the procedure) and and then if I am ok his assistant will come and take picture, give me the flyers about Botox side effects and adverse effect and ask me what medication I am using and if I am planning pregnant or pregnant.. Then I will pay and they will give me itemized receipt plus the credit card receipt... But he did not do many doctors practice... I will get appointment, go there, sign the Consent for Use of Botox and then will assess me without taking picture, then give me Botox then I will go to the front desk to pay and give me only credit card receipt.. I will ask then how much dose the doctor gave me and the first visit the front desk told me 40 units.. The second visit after maybe 4 or 5 months he did the same way..to my surprised I paid only $345.54 which I know he gave me lower dose.. the after few days no improvement and after maybe 4 or 5 days he gave me retouch for free but no idea how much he gave me and no effect..I fly to Canada and ask for second opinion to **. [redacted] Medical Corporation [redacted] Ave. Winnipeg Canada [redacted] and assess me told me how many units He will give me (41 units) and if OK ( maybe want to know if I can afford it normal like my other Doctors) I said yes and He's aware that I just had Botox in Hawaii few week before that.. the nurse come and explain to me everything and took me pictures and ask me to pay and gave me detailed receipt plus my visa card receipt and then they start the procedure and everything went well and I am very much satisfied. I file dispute against **. [redacted] and when he call me he said he will give me free Botox once I come bac
