Subject: ***Dear ***,This letter is in response to *** ***’ complaint #*** Mr.*** received treatment twice in our office for check-ups only in April and January He made and failed to show up for his appointments on 7-24-2014, 3-1-and 3-8-
Because of his failed appointments we charged him a fee of $which has subsequently grown to $due to billing chargesThe fee is charged to educate our patients to the importance of keeping their appointments and to have them inform us in a timely manner if there are changes they need to make.Due to the nature of this fee we have no interest in collecting the $from Mr *** and have since adjusted his balance off of his account The reason he has been receiving subsequent bills and recurring billing charges is due to our practice management software instructions Until we are notified of our patients intent we do not make changes in their individual account I cannot speak for what Mr*** claims to have happened in regards to his attempts to contact us But his past history of failed appointments leads us to question his statements.Again, we have adjusted his balance off of his account and he does not owe us anything.Please let me know if you have further questions.Sincerely,John U***, DDS
