examination, Mr*** noted that he had not been to dentist since 2003. Calculus buiwas visible on teeth during visual examination. Moderate bleeding occurred during periodontal probing; probing depths ranged from 4mm-7mm The reading of full mouth x-rays showed subgingival calculus and vertical bone loss. The patient was diagnosed with early to moderate active periodontitis type III. Periodontal disease, or periodontitis, is inflammation around (a) tooth/teeth. The gums in the affected area(s) pull away from the teeth and form spaces (pockets) that become infected, collecting bacteria and debris, that can destroy supporting tissue and bone leading to tooth shifting and tooth loss. The reviewing dentist’s decision is contingent on criteria, i.e., levels of “acceptable” bone (loss) deterioration, set forth by the insurance company for deciding payment approval. Dr***’s philosophy of care with regard to bone loss is that there is no acceptable level of loss. It is Dr***’s standard to treat this infection in its earliest stage, controlling the infection before irreversible damage occursEarly intervention can minimize infections and consequences caused by the bacteria, so the “acceptable” level of bone deterioration set by the insurance company does not occur In regards to the *** *** case, we take our patient’s complaints and concerns very seriouslyPlease let us know if we can be of any further assistanceThank you *** ***
