Re: Complaint ID #***To Whom it May Concern:This letter is in response to the complaint received on 08/20/2015, regarding patient *** ***.Blue Shield of CA was contacted today regarding the claims for date of service 05/19/and 06/09/2015, have attached a copy of the EOB that clearly
shows the patient's responsibility for the 06/09/visit, which is a total of $while the other service date of 05/19/is pending at this time.In regards to the estimate of cost that was given, we cannot guarantee what insurance will pay on the claimWe can only give an estimate based on the information that we receive from the InsuranceI have spoken to *** regarding her account and informed her that I would help her with the balance, but only once all claims have been receivedThe original claim for 05/19/was sent and denied on 06/01/and a follow up was done with the Insurance regarding the claimWe are toid the deniai was due to them not being able to locate the patient in the systemThe claim was resubmitted for a second time on 06/09/According to her insurance, they have nothing on file for the date of service, even though we submitted the claim a second timeAll information was verified and resent to the insurance company for a third time today.To resolve this situation, we are going to credit the patient's account, which is her due portion of $At this time, we are still waiting to receive payment from patient's insurance claim as they have yet to properly process our 3" attempt at getting this claim properly processedWe appreciate your involvement and response in this matter and we also hope to resolve this case and move on

Review: 05/2015 I went to Dental Oasis "D.O." for a teeth cleaning and xrays. Upon completing my appt & checking out I was informed that I owed $51 ($25 for my deductible and $26 for out of network ins. charge). At this time I was told my ins. company would cover the remainder of the balance. ~3 wks later I received a bill from D.O. seeking payment for the full balance for this visit. I spoke with a D.O. rep & was told my ins. company wouldn't cover the cost of my cleaning/x-rays. I called my ins. company & confirmed that they would cover the balance owed & that D.O. was submitting the bill incorrectly. I informed D.O. of their mistake. 60 days after my initial visit I received a 2nd bill stating I still owed the full amount for my cleaning. I again spoke with a D.O. rep and was told my ins. company wouldn't cover the balance. I again called my ins. company and confirmed that they indeed would cover the remaining balance and that D.O. was still submitting the bill incorrectly to them.

Also, at my cleaning/xrays it was determined by the D.O. dentist that I needed 2 fillings. Upon checking out I informed a D.O. rep that I would not have my fillings at their office bc it was out of my ins. network. She informed me that whether I had my filling done in OR out of network that my ins. comp would only cover $145 towards the filling and that I would only pay $8 more by having my filling done w/ them compared to an in network dentist. Upon hearing this I scheduled an appt to have 1 filling. After the filling work was complete I was informed by the front desk that I was going to owe more than initially told, but was not given a balance & was told she was going to bill ins. 1st then would send me a bill for the remaining balance due. I then received a bill for the full cost for my cleaning/xray/filling.

After this 2nd billing issue I went to my old dentist for the 2nd filling I was told I needed by D.O. Using the D.O.'s xrays and a new set it was determined no filling was needed.Desired Settlement: Had the Dental Oasis staff correctly informed me of the true cost that my insurance company would cover when having the filling work at their office I would have chosen to have the work done by my old dentist. Had I done this I would only have had to pay $~80-$100 for the first filling. I'm also worried that I didn't need the first filling due to the fact that I didn't need the second filling that Dental Oasis recommended that I needed.

My desired outcome is to pay $80-$100 to Dental Oasis for the filling and for them to correctly submit my teeth cleaning/x-ray bill to my insurance company and stop trying to charge me for the full balance

We can only give an estimate based on the information that we receive from the Insurance. I have spoken to [redacted] regarding her account and informed her that I would help her with the balance, but only once all claims have been received. The original claim for 05/19/2015 was sent and denied on 06/01/2015 and a follow up was done with the Insurance regarding the claim. We are toid the deniai was due to them not being able to locate the patient in the system. The claim was resubmitted for a second time on 06/09/2015. According to her insurance, they have nothing on file for the date of service, even though we submitted the claim a second time. All information was verified and resent to the insurance company for a third time today.To resolve this situation, we are going to credit the patient's account, which is her due portion of $268.60. At this time, we are still waiting to receive payment from patient's insurance claim as they have yet to properly process our 3" attempt at getting this claim properly processed.
