To Whom It May Concern: Mr*** was referred to an endodontist (Dr*** ***) on 2/3/for #Notes showed on 2/3/that there was pathology on the apex on #I recommended a root canal or an extraction, and patient chose endodontics and a referral was givenMr*** received the root canal therapy on 2/12/by Dr*** ***According to *** ***’s statement that procedure cost him $Dr*** billed him the $900.00, not our office (*** *** ***) After the root canal was done, it was decided that it was best to put a filling in #Mr***’s out of pocket portion for the filling was $We did see Mr*** on 8/4/for a routine exam and prophy (cleaning)No radiographs were taken at this timeAt this time there were no remarkable comments on #On 2/9/16, on Mr***’s next exam and prophy (cleaning), we did take x-rays and that is when the root fracture on #was notedIf the root was fractured before the time of the root canal therapy, Dr***, would have seen that and recommended extractionIt is extremely difficult to predict if or when a tooth may fractureWe recommend the best treatment to extend the life of the toothI understand Mr***’s frustration and I would be more than happy to refund the $that he put towards the tooth at my officeIf Mr*** would like the $back that he put towards the root canal therapy, he would need to go back to Dr***’s office and request the money from there.Sent on: 3/18/5:34:PMSent by: ***

Please add your rejection comments below. My Dentist had referred me to Dr. [redacted] DMD, He told me I would loose the tooth if the procedure was not done. I did have it done. I paid be credit card. He did show me the picture on the computer. I was told it was fine and to return in 6  months to be examined by a Doctor [redacted]. The tooth was fractured and I was told it would have to be pulled. The tooth is now gone and the money I spent for the root canal. I would like a refund.

We received a complaint from our patient Mr. [redacted]. The tooth that is in question is #7. Which had a root canal on it on 2/12/15, which was done by Dr. [redacted] at [redacted], MA [redacted]. The notes from the day Mr. [redacted] is questioning is from 2/9/16, is as follows:...

RMH: meds simvastatin cislasdalol asa RDH: no c/o, no bleeding when brushing/flossing, flosses daily OCS: neg exam 139, no tobacco, 4PAs taken OH: patient forgot to bring his partials to clean; sulcus scale, light to moder sulcular plaq/no calc and moder bleeding; full perio charting reveals severe recession esp mand lings and #7L is 8mm, 139 (Dr. [redacted]) feels there`s a fracture in the tooth wh/ is presently asymotomatic per patient, decay free per 139 (Dr. [redacted]) who needs partial to see if can add a #7 to it Review HC: sugg bringing his partials in to have them soaked in T&S remover at his convenience, review longer brushing NV: 6MRC and patent to call if #7 becomes symptomatic Initials: [redacted] and Dr. [redacted] Dr. [redacted] diagnosed root fracture #7 (endo treated 2/2015) and recommended extraction. Patient did not want to extract #7 because he did not have any symptoms. Mr. [redacted] did not bring partial denture with him to exam appointment, so it was impossible to determine whether a denture tooth could be added to his existing partial or if a new RPD (removable partial denture) would be needed to replace #7 after extraction.
