To Whom It May Concern, I am writing you this in response to a letter we received from your institution regarding a concern of one of my patient. My team and I try our best to make sure all our patients are happy and well treated in our facilities every day. My intention in writing this letter is to...

try to explain what has taken place in my office regarding this patient and her kids. Based on her statement, it seems there are several issues that I am very surprised to read in her statement. As part of my company policy, we always call our patients next day to make sure they are doing OK after their treatment. It is the duty of my front staff to call them and make sure they are satisfied with our service. No concerns or disappointment was EVER expressed, when my staff talked to this patient regarding herself or her kids. Regarding all our treatment, we do require a signature before we proceed. This is true regarding all our patients, including this particular case. In our office one of the staff spends as much time as necessary to explain the proposed treatment. Once the patient has no concerns or question they have to sign the consent form which gives us the permission to proceed with the treatment plan. As with all our patients, she was also proposed the treatment plan. The understood, agreed and signed for it. We will be happy to provide you with copy of her signature and the forms. I am very surprised that in her statement she stated “never asked me for, treatment option". All her options were explained to her and she did sign the explanation of treatment option. I will be happy to provide you with such documentation if there is a need. All the treatments were done for her and her kids. Everything has been charted and can be presented to the appropriate party if necessary. She expressed concern about being in the room with her kids. She came to our office with 5 children. We were as accommodating as possible. Our rooms are small and does not accommodate me and my assistance, patient and 5 other people. All this was explained to her and she seemed very happy at the time of her visit. Also for the safety of the children I could not have whole family in during the treatment. My patient was in the room with her children during the x-ray as well as examination. The silver crown did fell, as they do sometimes. She waited several weeks before she came back to our office. Even though at the time of the treatment, it was explained to her should such thing happen come back to our office right away. She ignored our instruction As far as refund, we have done our treatment. We will be happy to take care of any problem gladly. I should also inform you, as of now she still has not paid her out of the pocket portion of the dental bill. This bill could have been sent to collection agency but for now we just want to make sure our patient is happy and content with our service. Again, she has sign a document in which she has agreed to pay her co-pay. It is a legal document that obligates her for payment.
