On 12/15/17, Mr. [redacted] received a comprehensive dental examination, x-rays and periodontal charting to assess the condition of his gums and teeth. recommended by the dentist and hygienist cannot be substituted with an adult prophy (healthy mouth cleaning) as that would not address the patient’s periodontal condition. The Laser Therapy recommended in conjunction with this treatment can be declined by the patient.
Mr. [redacted] may contact the dental office and request a new treatment plan which does not include the recommended Laser Therapy.
