It has been months I contracted *** to ask about the process of scultra, she told me that I would have to get an evaluation and see the doctor to see if I needed the productThe doctor approved and I filled out the application and they sent two jars of the product and I would have to pay 200$I paid and went to the doctor that applied the product, after the appointment the doctor told me to return in two weeks because the product is applied gradually, but *** told me to apply both bottles at one timeI told her that I would like to speak with the doctor because they are telling me two different things, she tells me apply both while the doctor says to come next weekI could no longer speak to the doctor and I want them to tell me where the remaining product is.TRANSLATED BY Revdex.com EMPLOYEE

Good afternoon, Thank you for the notice concerning [redacted]'s complaint.  I apologize for the delayed response as today was my first awareness of the complaint.   After reviewing the letter of complaint, one would understand why the patient would be angry OR one can...

surmise the story is fabricated.  I looked into patient's file for treatment record on April 7, 2017, doctor clearly charted how much he injected and located exactly where the product was placed. It is confirmed that Dr. [redacted] injected 2 vials of Sculptra that were sent to our office by [redacted]'s Patient assistance program.  Unfortunately, we can not disclose patient medical record/face chart due HIPPAA policy.  It is a common practice for the doctor to instruct his patients to schedule a follow-up appointment after seeing him for any treatment.  With Sculptra being a product that would require multiple treatments in order for the patient to reach ultimate result, Dr. [redacted] discussed that patient would need more vials to restore volume loss.  I would like to reiterate that patient DID NOT pay for the product, they were FREE of charge by [redacted] Patient Assistance Program, his responsibility was to pay for the injection fee.  I assisted  [redacted] throughout the entire process of 10 months as a courtesy to help from submitting documentation to apply for the product to processing prescription of the product in order for [redacted] to fulfill Sculptra vials.  After multiple attempts to call, fax to [redacted], we were happy to schedule the injection date for Dr. [redacted] to inject with a better than minimal service fee of $200. This is not the first time [redacted] has had Sculptra injected, so I think he would know the difference between 1 or 2 vials in his face. When I last communicated with [redacted], I was taken aback by his attitude after all the assistance he received from our office.  In summary, [redacted] received 2 vials of Sculptra on April 7, 2017, we did not withhold his product. It he believes he was cheated, I would recommend him to see another physician and I would be more than happy to forward his medical record to that office once I receive a Medical Record request.  I hope this response canvases a clear picture of what transpired. Please feel free to call or respond via email with further questions. Best regards, [redacted]
