I am writing in response to a letter we received regarding your customer and our patient [redacted] [redacted] was seen as a new patient at our office 5/4/2015.Prior to Mr [redacted] being seen in the room by Physician/Physician Assistant his insurance was verified by our front desk and he signed an insurance waiver stating: “All services rendered are payable on the day of the visitIf insured, as a courtesy, your claim will be submitted to your insurance; however, any co-pay, deductible or patient portion is payable upon the day of serviceI authorize payment of medical benefits to the assigned physician and the release of medical information necessary to secure the payment of benefitsI understand that if my account becomes delinquent, I will be held responsible for the reasonable attorney's fees, court costs, and collection costs.” It is up to Mr [redacted] to know his insurance and what percentage he is responsible for regarding medical visits,After insurance was checked and Mr [redacted] was examined, it was determined that two skin biopsies would need to be performed, as well and cryotherapy for precancerous lesionsPrior to biopsies Mr [redacted] was asked again to sign a consent form understanding the risks, benefits, and alternatives of the surgical procedure of having a skin biopsyMr [redacted] 's insurance was billed accordingly, for an office visit skin check, cryotherapy, and two skin biopsiesPatients insurance was billed and payment made accordingly to contract with patient insuranceHis insurance pald a percentage and now Mr [redacted] is responsible for his percentage.Mr [redacted] was notified 5/12/of positive skin cancers on the phone, and a letter was sent on 5/28/regarding no follregarding his positive skin cancers.Mr [redacted] was sent to collections after statements were sent regarding his outstanding balance.Through [redacted] in our billing department it was communicated to patient that his insurance was holding him responsible for a percentage of his procedures done at the office, Mr [redacted] feels his insurance should pay it all, unfortunately that is not the plan he chose when selecting his insurance
