complainant on 07/10/The original charge was $A claim was submitted to her insurance company on 08/08/On 08/21/2014, the complainant's insurance company responded that they allowed the charge and applied the contracted allowed amount of $towards her deductible, meaning the complainant was responsible for paymentWe began billing Ms*** on 08/25/We sent statements on 08/25/2014, 09/26/2014, 10/27/A days past due letter was sent on 11/21/2014, and a final notice was sent on 12/05/None of the statements were returnedOn 12/17/2014, we received notice from the post office that the complainant had relocated and the latest statement had been forwarded to the new addressOn 01/07/2015, our office contacted Ms*** via phone, explained that the charge had been applied to her deductible, and sent one more statement at her request to the address she verified during the call.We received no further contact from Ms*** and the account was turned over to a collection agency, *** *** in February 2015, in the amount of $(this included rebilling fees of $2.42, $2.42, and $that were assessed in October, November and December after several months of billing the patient)We did not hear from the complainant again until we received your letterhave also enclosed a copy of the history of the Ms***'s account from *** which indicates the many attempts they made to contact the patient and the breakdown of the additional fees that were added to her account, after we turned the account over to them.In summary, we contacted Ms*** multiple times, both via *** and over the phone, and she was aware of the amount due on the account before it was turned over to collections.SincerelyStacey M*** Administrator*** ***
