This letter is in response to the complaint submitted on
August 29th by Joshua [redacted] who resides
at [redacted]. Mr. [redacted]'s complaint indicates
that he has been receiving mail at his home address from Dermpath Diagnostics
which is addressed to Anne [redacted]....

He states that he does not know this
individual and she has never lived at that address. He also states that he
contacted our office two months ago and gave this information to whoever
answered the phone.After reviewing our patient records, we found that we do
not have a patient with the name Anne [redacted]. After pursuing this issue
further with our corporate billing office, it was determined that there is a
patient by the name of Anne [redacted], but she is a patient of Dermpath
Diagnostics located in [redacted], New York.Our Dermpath Diagnostics Wisconsin location did not mail
anything to Mr. [redacted]'s address. Dermpath Diagnostics is a division of Quest
Diagnostics with multiple locations within the United States. Each location is
its own entity and is not affliated with the other locations when it comes to
mailing patient correspondence or billing statements. The information regarding
Anne [redacted]'s address has been forwarded to the Dermpath Diagnostics [redacted]
location. Please review this complaint, so that our company name may be cleared
of any wrong doing.
