Dermpath Diagnostics

I never requested a skin cyst removed to be sent to the lab and here I am with a $77 bill statement for something I did not request. I also noticed there is the wrong insurance information on the statement, when trying to call 866-312-7133 during normal business hours (11:00am), the answering service states to call between 8:30am - 5:00pm and there is no chance to leave a message. The only option this company gives you is to pay online. Why should I pay online for a service I did not request? Why can this company have customer service people available to discuss billing errors?
