The first and only documented contact we have with Mr*** regarding a refund was when he sent his "unhappy" email on 4/24/Our front desk person responded to his email stating she would have our insurance coordinator look into it on Monday (4/27) when she was in the office
After our insurance coordinator reviewed the account, a refund check was issued to Mr*** and mailed on 4/29/His account is currently at a zero balanceThe Complaint letter was received in our office on 5/4/15, so the matter was resolved prior to receiving the letter
