Thank you for bringing this to our attention.  I have reviewed the complaint. Our self pay accounts are handled by an outside vendor [redacted].  On 11/13/15 the patient called and spoke with a rep name [redacted]. She indicated she wanted to make payment in full and requested an itemized bill. She did...

provide her insurance information during the call. The patient paid the biill in full by credit card.  She did provide her insurance information, but the rep assumed that since she paid the claim in full, the patient would work with her insurance company to retrieve payment. The rep should have forwarded the insurance information to the Central Business Office for billing.
Patient called again on 12/7/15 and advise the [redacted] rep [redacted] taht she was informed that her insurance company did nothave a claim on file. At this time [redacted] should have reverified/confirmed the patients insurance information and submitted to the CBO for billing. However, this was not done.
Patient called again on 12/14/15. Patient was advised by [redacted] rep [redacted] that we have insurance information on file. However, she had decided to pay in full and had requested an itemized bill. At this time [redacted] requested that this insurance information be forwarded t the CBO for Billing. on 12/15/2016 [redacted] at the CBO recieved the billing information.
Unfortunately, [redacted] fail to submit the insurance information to the Billing Staff to send the claim to the patient's insurance.  As of 2/18/2016 a claim has been submitted to that patient's insurance complany. [redacted] 02903  ###-###-#### Policy#[redacted]. Claim is currently in process. Once Payment is received from the payor the patient will receive a refund.
[redacted] has used this account as an educational tool to [redacted] and [redacted] for handling the incorrectly. The CBO has also address this account with [redacted].
