I am writing in response to the complaint filed on 1/23/2018, assigned ***Contrary to the complaint filed, our office did file claim to the patient's insurance company in a timely mannerPatient was seen in our office on 6/19/and the claim was filed and accepted by the insurance on
and denied on 6/for 'PR 31- patient not found"Our research proved that the insurance coverage information provided by patient was invalid incorrect ID number and date of birth, hence the denial of the claim by the insurance companyAs an oversight on our part, we should have reached out to the patient to correct the error immediately and refileThe insurance company later provided us with the correct information to refile the claim for payment, after patient reached out to them to question tie denial of the claimThe claim has since been refiled in January, pending adjudication.Our office has also reached out to the patient to resolve the issuePatient was very undersbnding and looking forward to the issues being addressedPatient agreed to pay their portion of the claim, if responsibleHowever, if not responsible due to our oversight, we have also agreed to write off the patient's balance.Thank you for bringing this to our attention

I JOINED THE DIET CENTER ON 10/23/13. I WAS SOLD A WEIGHT LOSS PROGRAM. ON THE PAPERWORK, IT SAID THE FOOD WAS $45.00 A WEEK. I ASKED THE SALESPERSON DID THAT INCLUDE EVERYTHING I NEEDED. THE REPLY WAS YES.SO I SIGNED UP FOR THE PROGRAM. HOWEVER, IT WAS IN FACT NOT TRUTHFUL. THE $45.00 IS ONLY FOR THE BASIC 2 BOXES OF SHAKES NOT THE 4 BOXES OF SHAKES NEEDED TO PROPERLY MAINTAIN THE PROGRAM. IT ALSO DOES NOT INCLUDE THE CHIPS, VITAMINS. I DISPUTED THE FOOD PRICES AND TOLD THEM I WAS NOT GIVEN THE PROPER INFORMATION. THE DIRECTOR OF THE [redacted], MS CENTER TOLD ME TO COME IN AND WE COULD TALK ABOUT THE PROGRAM. SHE WOULD GO OVER IT AGAIN WITH ME. I DISCUSSED IT WITH THE MATTER WITH THE DIRECTOR AND WAS OFFERED NO SOLUTION. THE INFORMATION I SHOULD HAVE BEEN PRESENTED ON THE FRONT END. ON 10/28/2013, I CALLED AND ASKED TO SPEAK TO THE PERSON WHO SOLD ME THE PROGRAM AND SHE IS NO LONGER WITH THE COMPANY. ON 10/29/2013, I WENT INTO THE DIET CENTER IN [redacted], MS TO GET A REFUND AND WAS TOLD THEY DO NO GIVE REFUNDS. I WAS NOT PRESENTED THE PROPER INFORMATION IN THE BEGINNING REGARDING FEE SCHEDULES AND THE REQUIREMENTS OF THE PROGRAM. THEY HAVE UNETHICAL BUSINESS PRACTICES. THEY ALSO HAVE A BURDEN OF RESPONSIBILITY BECAUSE THE SALESPERSON WORKED FOR THEM AND SOLD ME THE PROGRAM WITHOUT GIVING ME THE PROPER INFORMATION. WHICH I HAD NO WAY OF KNOWING, UNTIL I HAD TO BUY FOOD FOR THE SECOND WEEK. THEIR BUSINESS PRACTICES ARE VERY UNPROFESSIONAL.Product_Or_Service: DIET PROGRAMDesired SettlementI WANT A REFUND ON THE PROGRAM AS WELL AS THE FOOD. I WOULD HAVE NEVER SIGNED UP FOR THE PROGRAM IF I HAD BEEN GIVEN THE PROPER INFORMATION IN THE BEGINNING.

Constant phone marketing persists years after being asked multiple times to be removedI visited diet center a couple of times years ago. Since quitting this company calls and [redacted]s constantly. I have asked in person, I have asked via phone, I have asked via [redacted], and I have asked via writing to be removed from their contact list. This has persisted for years. Usually I just block the number or the [redacted] address but now they're using all 0s for the call number or otherwise obfuscating. They call all hours and week day or weekend with no option to speak to a representative. I'm beyond sick of it. I'm close to changing my number, but it's ridiculous that it should come to that.Desired SettlementStop calling and emailing me. Never ever contact me again. Ever. Business Response Good day. Thank you for your feedback regarding Diet Center. The paperwork submitted by [redacted] at the time of enrollment onto the Diet Center programmed asked for phone numbers, mailing address and an email address where she could be contacted to keep her accountable to losing weight and maintaining a healthy weight which includes follow up communication after participation in the Diet Center program. The form also asks "May we contact you here?". All fields on the form were marked "YES" and signed off by Mrs. [redacted]' and also includes her signature on file. We log all communications with clients, and do not have a record or Mrs. [redacted] requesting removal. Additionally, all emails sent by the Diet Center company provide an "unsubscribe" button for consumers to self-unsubscribe at any time and a phone number and physical mailing address for any correspondence that need attention. We had not received any such communication at this phone number or address. Here is what we can do for the consumer:-request our mailing vendor remove Mrs. [redacted] from future mailings. Please keep in mind it is managed by the [redacted] and at times they do mail overweight individuals based on health data and demographic data provided by multiple sources. Although Mrs. [redacted] will be removed from our mailing list, we do not control the mailings or demographic information she would qualify for. -ensure Mrs [redacted] is removed from our call list. Since an email address was not provided in the feedback ofcorrespondence from the Revdex.com, Mrs. [redacted] will need to take action in unsubscribing from any emails she receives. We wish Mrs. [redacted] health and happiness at her goals and a lifetime of maintaining her weight. However, saying there is no 'recording' of me requesting removal is just nonsense. I have asked numerous times on the phone for the caller to remove me from the call list, as I was no longer a customer. But of course there is no way for me to prove this since I don't 'log all communications' myself.

They have not removed my name from mailing list after numerous requests.I visited their [redacted] location twice but never signed up for their weight loss program. I continually receive cards in mail of special offers. I have called more than 4 times to be removed from their mailing list. BUT I keep getting these offers and with all the bogus mail I receive it is a lot of trouble to remove labels to be destroyed. Desired Settlementplace this written complaint on their record and forward them a cease-and-desist statement for my benefit.Thank youBusiness Response Diet Center uses the Commercial Appeal to send postcards on their behalf. The [redacted] has been notified to remove Mrs. [redacted] from our company mailing list. However, the [redacted] send postcards to demographic consumer lists and Mrs. [redacted] may still appear there if she has ever purchased another weight loss product from any company (i.e. [redacted] etx) in her consumer lifetime. The [redacted] uses general consumer mailing lists provided by companies such as [redacted] for any general mailers and Diet Center does not control those consumer lists. My intent was to be as transparent to Mrs. [redacted] as I could be. We have removed her name from our internal mailing list. However, when advertising is sent by the [redacted] they often use mailing lists from third party companies. These targeted mailings will go to people who have purchased weight loss services or products in the past. I felt that not sharing this information with Mrs. [redacted] would not give her a clear response. We will not purposely mail her anything from Diet Center, but if she has purchased any weight loss product in the past, there is a chance that she may receive something due to her appearing on mailing lists sourced from other companies.
