on 1/4/16.*** ***'s mother, Netria Marlin, was a resident in our facility from August Th, thru September 11, 2015, I am sorry to say that I did not receive the letter Ms *** states she sent to the facilityI reviewed her file and did not find any notation of phone calls regarding her billI am glad to have this opportunity to assist in resolving her concerns.Prior to admission Ms*** was informed of our Room and Board rate for the Augusta's Cottage floor of $185.00/dayIt was explained to her that there could be additional charges that would post on the following month's bill for Items such as therapy, gloves and incontinent supplies to name a fewMs Marlin received physical therapy while staying with us which accounts for the (B) Coinsurance / Co-pay that is listed on the statementMedicare was billed for the physical therapy and this is simply her co-pay for those services, The Specialty Equipment included on the statement is a specialty mattress that was rented for Ms Marlin in an effort to help with her trouble sleeping and resistance to sleeping in her bedWe do make every effort to bill insurance for services and equipment, however per our Admissions documentation if payment is not received from the insurance company within days the resident is billed.We have contacted our specialty equipment vendor for more specific details regarding the mattress that was orderedI would welcome any communication Ms *** would like to provide me to help us ensure that if any billing errors were made, they are promptly resolved.I look forward to speaking with Ms *** and can be reached at ***; Mon-Fri8:00am-4:00pm. After careful consideration of *** ***'s complaint we have decided to remove the $43700charge to Netria Marlin's account (#3111) for the Specialty bed that was rented for her useThis leaves an outstanding balance of $of Medicare Co-Insurance for Physical Therapy services provided toNetria Marlin during her stay.We apologize for any communication errors that may have allowed Ms***'s concerns to not behandled within an acceptable timeframeWe sincerely hope that this resolves Ms***'s concerns and is an acceptable resolution.Please contact me with any questions or concernsI appreciate your time and efforts in facilitating this matter.Thank You,Stacey ***Business Office Manager

Regards,[redacted] I was at this place so much it was if I lived there and I do have pictures of her room as well as the badly needed to be replaced mattress, along with lots of others, I took pictures daily of my mom as well, they never provided a special mattress of any kind for my mother, They were sent several letters asking to explain this bill one letter could have gotten lost 4 no way as well as the others asking about her medicines they kept and her clothes, I feel they had no reasoning to this charge and it took 5 months to come up with something that they may could get away with, they also knew when my mom was to be released and I paid a final bill, they also never billed her insurance for this charge, which I will send the medicare rep who is over seeing this claim and her other insurance a copy of this e-mail
