Diversicare Healthcare Services, Inc.

My mother was in the [redacted] facility on [redacted] Blvd, Tupelo, MS. Recently, the name of the facility was changed to Diversicare. My mom was placed at the facility due to dementia. My mom was not properly watched by the staff and as a result, she was able to cut the Wander Guard security bracelet from her arm and walk out of the front door at supposedly approximately 3:30 a.m. She walked down a very busy two-lane street, [redacted] Blvd. 18-wheeler trucks constantly travel this street. My mom could have been killed. We received a call after she was found. However, we should have received a call when she was first discovered missing. To be honest, I really don't know at what time my mom left the facility. I am only going by what I was told from a staff member who called my house once my mom was found. When my mom was found, she was not transported to the hospital as a follow-up to make sure that she was in good condition after such a traumatic event. On another occasion, my mom had taken her "security bracelet" off and no one at the facility knew that she had taken the bracelet off. They were only aware of the bracelet being off when I screamed out of fear and told someone that she was walking out of the door. One time, the police was called on my mom because she was being transported to the hospital. I was told by the nurse that this was thee procedure they used when a patient was being combative. When the police arrived, I proceeded to tell the officer that my mom had dementia and the nurse rudely interrupted and said to the police that there were a lot more issues going on with my mom besides the dementia such as a lot of manic diagnoses. She named about three words that started with "manic". On another occasion, my mom ran from the facility toward the busy street. She was apprehended by staff members. A couple of the staff members did not appear to be properly trained to deal with dementia patients. I witnessed a nurse openly disagree with what my my mom said, which caused my mom to become angry and agitated. The facility administrator called me one day and said that my mom was improperly placed. Keep in mind that my mom had been at the facility for over a year. She told me that I needed to come and get her and take her to the new facility. She indicated that they had found a bed for her there. I was not given any time to make arrangements for coverage at my job. My mom arrived at the new facility with none of her medicine. The social worker at the new facility had indicated that she told [redacted] to bring my mom's medicine and still they brought her to the new facility with no meds. My mom still had the security bracelet on her arm from the other facility. My mom was emotionally distraught and said that she wanted to die because they had "thrown her out" of the facility.
