My mother was in the [redacted] facility on [redacted] Blvd, Tupelo, MSRecently, the name of the facility was changed to DiversicareMy mom was placed at the facility due to dementiaMy mom was not properly watched by the staff and as a result, she was able to cut the Wander Guard security bracelet from her arm and walk out of the front door at supposedly approximately 3:a.mShe walked down a very busy two-lane street, [redacted] Blvd18-wheeler trucks constantly travel this streetMy mom could have been killedWe received a call after she was foundHowever, we should have received a call when she was first discovered missingTo be honest, I really don't know at what time my mom left the facilityI am only going by what I was told from a staff member who called my house once my mom was foundWhen my mom was found, she was not transported to the hospital as a follto make sure that she was in good condition after such a traumatic eventOn another occasion, my mom had taken her "security bracelet" off and no one at the facility knew that she had taken the bracelet offThey were only aware of the bracelet being off when I screamed out of fear and told someone that she was walking out of the doorOne time, the police was called on my mom because she was being transported to the hospitalI was told by the nurse that this was thee procedure they used when a patient was being combativeWhen the police arrived, I proceeded to tell the officer that my mom had dementia and the nurse rudely interrupted and said to the police that there were a lot more issues going on with my mom besides the dementia such as a lot of manic diagnosesShe named about three words that started with "manic"On another occasion, my mom ran from the facility toward the busy streetShe was apprehended by staff membersA couple of the staff members did not appear to be properly trained to deal with dementia patientsI witnessed a nurse openly disagree with what my my mom said, which caused my mom to become angry and agitatedThe facility administrator called me one day and said that my mom was improperly placedKeep in mind that my mom had been at the facility for over a yearShe told me that I needed to come and get her and take her to the new facilityShe indicated that they had found a bed for her thereI was not given any time to make arrangements for coverage at my jobMy mom arrived at the new facility with none of her medicineThe social worker at the new facility had indicated that she told [redacted] to bring my mom's medicine and still they brought her to the new facility with no medsMy mom still had the security bracelet on her arm from the other facilityMy mom was emotionally distraught and said that she wanted to die because they had "thrown her out" of the facility
