1. I thought I was on the SSA website to get a duplicate Soc Sec card, and paid the "fee" for filing.
2. I received an email 4 days later from a private individual that alerted me to the fact that I was NOT on the SSA website, that SSA does not charge for their services, and that he, too, had been a victim of scamming by applyeform.com and that I should notify my bank of the charges.
3. When asked how he knew that I had used the same service, he said that when he went back to applyeform.com to request a refund, they put my email on his refund form.
