Review: In regards to the dental services provided 26 SEPT 2013 and 09 NOV 2013: During a follow up visit my wife and I were both told that we did not have an appointment for this day in 9 November 2012. We had made the long trip from our home and had the appointment card that was filled out by Mrs. [redacted] confirming date and time that we had arrived were correct. Though an apology was given this did not make up for the over three hours of commute for no service as we live in [redacted], NC. I chose Jesse Wall Family Dentistry as I had fantastic experiences in the past while operating with the military under active orders. I continued to make the one hour and forty minute, one way trip to their office even after I changed my insurance to [redacted] in the fall of 2012. I received a bill from Family Dentistry of Danville (dated 21 NOV 2013) without the claim being submitted to my insurer. I contacted the office and was told: "We only submit the claims as a courtesy to the patient and it is not our responsibility" but they would go ahead and submit this claim for me. I contacted [redacted] Dental, my dental insurance company, to make sure that it had been submitted and they informed me that this may take up to sixty days to process. I received a voice mail from their office at the end of March 2013 about a balance that [redacted] did not pay. I returned their call to discuss my concerns with the billing amount and was told by Mrs. [redacted] that she would talk to Doctor Wall and give me a call back the following week with an adjustment as she had regretted the misunderstanding. Since that time, I have been in contact with Jessie Wall's office (16 APR 2013) and a settlement was expressed over the telephone (though the amount was not discussed, Mrs. [redacted] had just said "we will send you a bill of what we charge family to make up for the mistake.") However, when I received the requested bill it was for an amount with only a minor difference amount owed plus interest. I have since left two voice mails (20 May and 3 June 2013) at ###-###-#### without a call back or any other communication from their office. I also called several times without leaving messages and I had sent a certified letter that was received and signed for by [redacted] (20 June 2013) asking for a response. My calls and letter requesting a response from Family Dentistry of Danville, VA have gone unanswered. Finally, I scheduled an appointment with a local dentist (21 June 2013) ([redacted] Dental of [redacted], NC) as I am still in need of dental work. My new local dentist ([redacted] Dental) contacted Jessie Wall's office and [redacted] Dental was told that Jessie [redacted] Dentistry will not release my dental records.Desired Settlement: As I was promised by Jessie Wall's office- the services that I was receiving would be covered by my insurance, the assumed disregard for my request for a response by telephone, and lost time in driving to an appointment that Jessie Wall’s office failed to enter into their electronic schedule; I feel that an appropriate settlement amount is in order. I will agree to pay twenty-five percent of the amount that their office charged for the services that they assured me were covered by my insurer. (This may not include any fees, service charges, interest, etc. as this bill has not yet been presented to me with a correct amount) I feel this is fair considering had the services actually been covered, as promised, I would be responsible for only a small fraction of only a few of the services I had received. 1) To be re-billed for 25% of the alleged charges 2) My dental records must be released to [redacted] Dental Practice; , NC [redacted] ###-###-####

Dear Ms [redacted]- I am writing in response to your letter of July 3, 2013 dealing with item [redacted]. Some parts of this document are indeed true. Our office does treat people who are in the military who are not at present stationed on a military base full time. Although the lower fees paid and the extra paperwork involvedin doing so keeps many other offices from offering this service. Dr. Wall served in the military during the Viet Nam war and was a STAFF SGT when he was honorably discharged. At least in part due to his service experience, Dr. Wall has always wanted this office to offer any help possible to members of our military.Of course, service members do not pay for any treatment, therefore the actual cost of dental and healthcare come as a surprise to these servicemen in civilian life. In the 30 plus years we have been open in Danville,we have indeed had patients from long distances, some as far north as Winchester, VA and some as far southas Charlotte, NC. However, blessed as we are to have patients who make the long trips to see us, we would never refuse to see anyone who made such a long trip, no matter what type of schedule error was made. This was not done in Nov. or any other time! All patients who presented in this office were seen.We are restrained by HIPPA laws to even acknowledge whether or not a person is or has been a patient in our office. The same laws prevent us from sending records or radiographs before we receive the properHIPPA form from our patients. I can tell you that no patient who has followed these guidelines has not had their records sent to the requested new provider. Mistakes in billing can happen, but to my knowledge none have occurred this year . Insurance plans have not paid for treatment due to waiting periods, some plans only cover exams and parts of other care! Insurance is a complicated industry. We do not sell or manage any insurance for any purpose! Our office will assist patients in maximizing their insurance benefits when we can. The individual and/or their employer are the decision makers about what product to buy and how it applies. Our office does offer our extended family members a 30% discount on services. This "family" rate is not a small discount in our opinion. The family rate has been offered to some non family members of our5 practice on special occasions. We have never received an inquiry offering to pay 25% of an outstanding bill. We have received one written request for a 50 % reduction in one family's outstanding balance, this request was granted and a statement reflecting this was mailed to the family. Our office regrets that anyone is unhappy and has always tried to turn the other cheek.I wish the laws would allow us to respond to [redacted] with documented evidence. I have reviewed the response offer made by the business in reference to complaint ID [redacted], and have determined that this proposed action would not resolve my complaint. For your reference, details of the offer I reviewed appear below.My wife and I WERE in fact turned away, without service, for our scheduled appointment time in November of 2012. In which we were told, by Mrs. [redacted], that an error on their part was made and the date was not entered into the scheduling software used by Dr. Wall's Practice. I also pleaded my concerns, during this time, that my wife had changed jobs which disrupted her dental insurance, that was expiring in December. This was the entire purpose of getting [redacted], my wife, in to see the Dr. Wall; as the next available date was outside of our availability, our insurance coverage period, not to mention our time allotment required to make the long trip. I have been in contact, by telephone, with Mrs. [redacted] 27JUNE2013: In which I had discussed a bill to be mailed to me that reflected the discussed changes. Mrs. [redacted] added notations in a previous bill (ATTACHED dated 25JUNE2013) that suggested the bill would not be paid in full after the amount on this particular paper bill was paid in total. I expressed my concerns to Mrs. [redacted] and she continued by asking me to "trust their office" and "that they did not do business like this" My requests were for a simple bill to be mailed to me stating the charges, amount owed and that once the a particular amount was paid that all accounts would be resolved and satisfied. Mrs [redacted] agreed to send a bill that would reflect this; however she would be unable to release my records as there was still a balance (which was against their office policy) I have sent a written request for "Authorization to Use and/or Disclose PHI" with intentions to have my dental records released to [redacted] Dental Practice ([redacted], NC) this letter was sent on 28JUNE2013; There has NOT been any response from Dr. Wall's office in regards to my request or letter. It is important to point out that Dr. Wall's office has not returned my calls after leaving several voice mails requesting followup on my concerns. ENCLOSURERegards,[redacted]
