I contacted this agency in June 2012 to inquire about chiropractic services.(First I contact my health insurance provider, [redacted] to inquire about coverage for this type of care. I was advised that insurance did cover it, and I was given a list of providers.) I contacted Advanced Wellness Centre to schedule an initial exam, and asked again if my insurance would cover services. They reported that they do accept my insurance. I was emailed a packet that I was required to complete and submit on the date of my first appointment. Included in the packet was a "Medical Services Contract" that indicated that I would be responsible for all charges arising from treatment, and that I would be responsible for obtaining a referral for treatment if it is required from my insurance, and that I would be financially responsible for any services denied for no referral. I DID NOT sign this form. At my initial exam I showed the contract to the receptionist and explained that I was not comfortable signing it, as they told me that my insurance would cover these services. The receptionist said that it was not a problem because my insurance would cover the costs. I began chiropractic services, and came in 5 to 6 times total. Each time I paid a $25.00 co-pay. My last appointment was on July 19, 2012. On September 17, 2012, I received a bill from this agency for $720.00. When I contacted the agency to inquire about this I was informed that insurance was denied because I did not have a referral. I explained that I was never informed that a referral was needed, and I also explained that I had refused to sign the Medical Services Contract. I was told that all clients were informed of referrals being needed. I again explained I was never told this. It was suggested that I contact my PCP to try and get a back dated referral. I attempted to get this and was told by my PCP, [redacted] that this could not be done, as this is considered insurance fraud. I am being told that I am responsible for the $720.00.
