We have no cancellation form on file from when he stated he cancelled his membership.As per the cancellation section of the membership agreement it states the following:1) Only L.A. will be accepted. No Exceptions.2) Memberships must be cancelled in person at the store that bills the monthly payment.It also states the following:L.A. Tan to automatically charge $21.99 to my credit/debit account, checking account, or savings account for payment of my monthly VIP membership fee. This authority is to remain in effect until L.A. Tan receives a VIP Cancellation form filled out correctly, signed on premise and countersigned by management. Management will not countersign your cancellation form if it is incomplete or not filled out correctly. I have attached his signed membership agreement for the record. The most we can offer Mr. [redacted] is a one month $21.99 refund or send him a $50 gift card to be at his convenience.
