On we received information from *** *** *** stating the charges we submitted for services received on 5/ 17/was before the effective date of this patient's coverageWe have enclosed a copy of the provider explanation of benefitsTherefore, benefits are not avail ablePer *** *** ***, the patient's coverage was active from 7// 13-10/He did not have coverage on 51171lTherefore, the patient was billed for the visit. patient s past Clue account was sent to County Credit Adjustment Bureau on 1/24/14 for collection involvement in the amount of $205.00.
