Dear Revdex.com,Mr [redacted] ongoing complaint has been frustrating to address We have tried to be as accommodating as possible to Mr [redacted] complaint As your records should show, we have requested that Mr [redacted] sign a HIPAA release so we could accurately address his complaint, but he has refused to In light of his refusal, we have tried to directly address his allegations without revealing any of his medical condition/treatment Although we don't believe what we've provided violates HIPAA, as it only addresses his allegations, I respect his wish to remove our response At this point, until we are allowed to respond with full information, we consider this case closed and will continue to deal with it internally Should anything change on your end, please let us know and we well be happy to provide additional information.Thank you.Kirk D [redacted]

Complaint: [redacted] I am rejecting this response because: Douglss Prosthetics & Orthotics broke rules and regulations for HIPAA Patient Privacy Rules CFR Part and Subparts A and E of Part Douglass Prosthetics & Orthotics needs to RETRACT and delete their response to the Revdex.com which includes my private medical information with attachments sent to the Revdex.com and Douglass needs to CREDIT my account immediately Douglass Prosthetics & Orthotics did NOT have my permission to release any medical information Otherwise, I will file a complaint against Douglass Prosthetics & Orthotics for not protecting and keeping my medical information private with the Department of Health and Human Services, state and other appropriate agencies I request the Revdex.com delete Douglass's response and attachments to maintain my medical privacy Please confirm with me that the Revdex.com will not make my medical information public Douglass should have made this a no brainer and satisfied the customer Thank you [redacted] Sincerely, [redacted]

? Dear Mr***, ? Unfortunately, until we receive a release of information from Mr [redacted] , we are unable to address the full extent of Mr [redacted] 's complaint.? We did receive the letter you forwarded from Mr [redacted] stating we "can not? send you medical paperwork" ? At this time we are only able to address his returning the Wound Healing Sandals.? Yes the shoes were sent to us via USPS and were signed for as part of our regular mail.? Not until we opened the package did we realize the shoes were from Mr [redacted] .? Returning the Wound Healing Sandals in this way is not acceptance on our part that we acknowledge their return and will credit his account.? ? Until we receive the HIPPA release, legally I'm unable to address the exact nature as to why the Sandals cannot be returned.? ? Kirk D [redacted]

I want to file a customer/patient complaint regarding Douglass Prosthetics and Orthotics, Seattle, WAI have had foot ulcers that take months to heal with the use of skin graft surgeryDouglass Prosthetics and Orthotics furnished me defective orthotic shoes which are unwearable and unusable to wear after my recent skin graft surgeryI do NOT want to risk another foot injuryMy doctor recommended I return the shoes to Douglass Prosthetics and Orthotics as they were not as prescribedI returned the shoes to Douglass Prosthetics and Orthotics via US mail and they were signed for and ACCEPTED by Douglass back in MayI requested $credit to my account from both their June and July statementsDouglass Prosthetics and Orthotics has harassed me and threaten me with collections of $for the defective orthotic shoes they furnished meThis is the second time I have had a problem with a Douglass orthotic device, the first time Douglass furnished an orthotic insert tha
