Review: I called the office several times to request my medical records. It is critical that I have my medical records. I mailed to the office a letter to release my medical records and then called the office to confirm receipt of that letter. At that time I spoke to [redacted]. One month later from my original request, I have yet to receive my medical records which are critical to my health and well being.Desired Settlement: I would like to receive my medical records.

This letter is to confirm that the above named patient was under my medical care and called requesting information regarding her IUD. At the time of the request, the patients chart was unable to be found due to the chart being put in storage and missed place during hurricane Sandy. The chart has been located and the patient's records have been copied and mailed out to her home address. A message was left on her voice mail regarding this matter and I also spoke with you as well.
